The patient measures their blood pressure using a validated blood pressure device, taking two readings one minute apart twice a day over a 30–day period. The provider takes a minimum of 12 readings and reports them to the provider. The provider collects the data, averages the systolic and diastolic blood pressures, prepares a report, and discusses a treatment plan with the patient.
