The occupational therapist (OT) performs a re–evaluation focusing on changes in the patient's functional, environment, or medical status that results in a revised plan of care; re–evaluation includes updating the occupational profile to reflect changes in the patient's condition or environment that affect therapy or goals. This service is performed when there is a change in the patient's functional status that requires a change in the plan of care and typically takes 30 minutes of face–to–face time with the patient or family or both.
