Follow-up Evaluation for Sports Injury Recovery

The provider carries out a re–evaluation of an established plans of care for athletic training on a patient recovering from a sports injury who has had a documented change in functional status. The provider again uses a standardized patient assessment instrument (criteria agreed upon by a group of experts) and/or a functional outcome measurement tool to evaluate a patient and revises the management options, goals, and therapeutic interventions.
