Disease Education and Management Program

A nonphysician healthcare professional uses a standard curriculum to educate patients about their disease or disorder, for example, asthma or diabetes. The provider also provides training on how to manage it more effectively. This education and training service enables the patients and or their caregivers or families to effectively manage the disease.
