Behavioral Assessment and Care Plan Development

The provider, a physician or other qualified healthcare professional, identifies the adaptive or maladaptive behavior of the patient by administering several different types of tests in person; the provider scores the test, interprets the results, develops a plan of care, prepares a report, and discusses their findings and recommendations with the parent, guardian, and/or caregiver. Report this code for each 15 minutes that the provider spends in face–to–face time with the patient and non–face–to–face time analyzing past data.
